In PURE, higher egg intake was associated with lower composite risk among participants with prevalent CVD at baseline (>=7/wk HR 0.81, 0.70-0.95) but not among those without (P-interaction 0.24); the apparent protective subgroup association was not reproduced in the two secondary-prevention cohorts (ONTARGET/TRANSCEND).

Results were otherwise consistent when early events (first 24 months) were excluded, across geographic regions, and in those with prior diabetes. The prevalent-CVD subgroup was the sole heterogeneity signal, and its interaction test was non-significant.
